By signing above, I understand and acknowledge that as a part of my participation in the Salt Lake Senate Team Tryouts, there are dangers, hazards and inherent risks to which I may be exposed, including the risk of serious physical injury, temporary or permanent disability, and even death. Furthermore, economic losses and property damage may also occur as a result of participating in these team tryouts. I hereby voluntarily assume all risks of accident or injury to my person or property whether foreseen or unforeseen. I acknowledge that I am aware that it is possible to incur minor and major injuries while participating in these team tryouts including but not limited to strains, sprains and fractures. I also understand that more severe injuries could occur such as concussions, internal injuries and even paralysis. Therefore, I agree to adhere to all safety rules and regulations during my participation in the Salt Lake Senate Tryouts. In addition, by signing this waiver form I agree to indemnify Salt Lake Senate from any liability for personal injury or property damage arising out of my participation in the team tryouts. This waiver will remain in force throughout the duration of my involvement with Salt Lake Senate team tryouts regardless if another waiver is signed at a later date. I hereby release Salt Lake Senate, its employees, agents and representatives from any claim, liability demand, or suit of action which may arise out of personal injury or property damage sustained by me related to my participation in these team tryouts. Furthermore I acknowledge that this agreement will bind both myself, my heirs and assigns forever should any legal action arise due to my involvement with the Salt Lake Senate Team.
